We appreciate being selected to serve the medical and health care needs of the members of our community, and we will do all we can to provide the very best care possible. The staff of medical professionals at Lafayette Orthopaedic Clinic; physicians, nurses, certified surgical technicians, x-ray technicians and office staff, are all dedicated to helping our patients get well and live a healthy life. Our orthopedic clinic uses the most up-to-date equipment and technology available to help us do just that.

We take great pride in our staff's training, experience, knowledge and capabilities, and we want our patients to have the same confidence. We would like to introduce you to the services we provide to our patients, explain office policies and procedures and answer some of your questions. Please feel free to contact our office directly if you have questions, in that way we can better serve you.

 

What to Bring on Your First Visit

Please bring a list of your medications or the actual medicine container so that we may record this in your chart. We will also take a detailed history of the condition or injury for which you are being seen. Please have the date of injury, previous surgeries for this condition, as well as the name, address and phone number of any physician who has treated this condition. A copy of your insurance card will be taken at each appointment.

 

Billing and Insurance

The physicians participate in a variety of insurance plans. We encourage you to take the time to contact your insurance company to ensure that you will have the appropriate coverage. For your convenience we accept cash, personal check, VISA, Discover and Mastercard.

 

Disability Forms

We will complete forms for disability payments, loans, etc. Payment is expected at the time forms are dropped off at the front desk. Every effort will be made to complete your disability forms in a timely manner.

 

Prescriptions

Due to potential abuses, no narcotics are prescribed over the phone. If you feel you need more pain medication, make sure you ask the doctor at the time of your visit. Narcotics are not typically prescribed any longer than 7-10 days after a fracture or surgery. New prescriptions and prescription refills will not be handled after office hours or on weekends. This ensures the doctor has access to your medical chart.

  • Rehabilitation
    Short and long term programs to improve strength and mobility and to optimize recovery. The orthopaedic surgeon works with other health care providers in order to meet those goals.
