How do I request copies of my medical records from Unity Healthcare?
Unity Healthcare medical records are stored and managed through individual practices. Medical records can be collected in paper form or electronically, whichever you prefer.
There is a three-step process for requesting copies of your medical records from Unity Healthcare.
- Download the Health Records Request/Release Authorization Form. This form is standard across Unity Healthcare and can be used to request copies of your medical records at any of our practices.

- Follow the instructions on the left side of the form to guide you in providing the type of information needed. Completing the form in its entirety will help ensure timely processing. If you choose to fill out the form on your computer/mobile device, typing your name in the signature line provides consent to release medical records.
- Fax, email, or mail the completed Authorization form to the Unity Healthcare practice from which you are requesting records. Practices and contact information are listed below. If mailing, please include the practice name, address and “Attn: Medical Records” on your mailing envelope. If requesting records for multiple practices, please utilize Medical Records Contact Info at the top of the form below.
