You don’t need to be a Division I athlete to receive healthcare like one.  Our sports medicine physicians are on the cutting edge, providing the finest specialized care available to athletes of all ages and levels.  We have the knowledge and expertise to treat everyone, from the recreational athlete to the elite competitor, and from the pediatric athlete to the active senior.

Our medical team provides both non-surgical and surgical management of work related injuries to help your employees regain their strength and mobility, and get back to work.
Our focus is on early determination of the patient’s diagnosis, and our treatment plans are designed to achieve maximum medical improvement.  Our team is dedicated to providing effective communication and high quality solutions. Whether you are an employer, case manager, or referring provider, we have the expertise to meet your needs.
We have a dedicated Workers’ Compensation Case Manager, Katie McKinsey, who will help you navigate the changing and complicated guidelines related to workplace injuries.

What to Bring: Please bring the following to your appointment:
Appointment Times: Because our surgeons see hospitalized patients, emergency room patients and perform surgeries, there may be times when your doctor will arrive late to the office. We do our very best to stay on schedule while still serving the emergent and surgical needs of our community.  If you are going to be late for your appointment, please contact our office.
Cancellations: Please call at least 24 hours in advance to cancel an appointment.
Payment: We take the same insurance as other Unity partners. Â Some insurance plans require that you meet a deductible before they pay while others require a co-pay for visits. Â Please contact your insurance company to understand your financial responsibility. We collect co-pays at time of service. Â
Request for Paperwork/Medical Records: We complete all requests for paperwork (medical records, paperwork for disability, workers compensation, and FMLA) on a first come, first serve basis. Due to the volume of requests, we cannot process same day requests or move any requests in front of others already waiting to be processed.  Please submit your paperwork at least 15 business days prior to the due date.  For all medical records requests there must be a fully completed and signed Medical Records Release Authorization submitted with the request.  There may be a charge for completion of medical records release requests as allowable by Indiana Department of Insurance.
