At the Pain Care Center, we are committed to improving the lives of people with chronic pain. Our assessment, treatment and prevention services focus on enriching the lives of the people we serve.

Chronic pain can be complicated, and sometimes confusing.  There are often several ways to approach and manage your condition.  We promise that we will do everything we can to help you manage your chronic pain. Together, we’ll explore treatment options to reduce your pain levels. Our goal is to improve your quality of life and get you back to living your best life.

At Pain Care Center we integrate traditional and complementary medicine and use a multidisciplinary team approach to treat your chronic pain.

Our objectives are to:

  1. Diagnose and treat chronic pain syndromes.
  2. Reduce pain and increase comfort.
  3. Increase the ability to manage pain and related problems.
  4. Increase physical capabilities and activities.
  5. Support the return to productive activity.
  6. Optimize the appropriate use of medications.
  7. Increase coping, self-care and stress management skills.
  8. Improve psychosocial adjustments and quality of life.

 

Chronic Pain

Sometimes chronic pain can begin without any obvious cause, but for many people, it starts after an injury or health condition.  Chronic pain is defined as a pain that lasts at least 3 months. It can affect your daily life and your mental health. Pain symptoms can range from mild to severe. It can continue daily or come and go.  The pain can feel like aching, throbbing, burning, shooting, stinging, soreness, or stiffness. Sometimes pain is just one of the symptoms.  It can also affect your energy level, change your appetite, affect your sleeping, or make you feel weak. Chronic pain can interfere with your daily life.  It can affect your self-esteem and cause you to feel angry, depressed, anxious, and scared. Working with a pain specialist, we will help you find relief from your pain so it won’t keep you from living your best life.

We appreciate the opportunity to care for you. Our staff is made up of qualified professionals, who work together as a team to bring you the highest quality treatment in a warm and caring setting.

Please review the following guidelines for Pain Care Center and its patients.

Arrival Time: Please arrive on time for your appointment.  In fairness to all other patients scheduled that day, we may have to reschedule your appointment if you are more than 15 minutes late.

Appointments vs. Walk-ins:  Patients must have an appointment to be seen. We do not take walk-ins.

Payment: Some insurance plans require the patient to pay a co-payment or co-insurance, which is due at the time of service. Please be prepared to pay at your appointment.  For your convenience we accept cash, in-state checks, and all major credit cards.

Cancellation Penalties: All appointments require a 24-hour cancellation notice.  If you do not call to cancel your appointment at least 24-hours ahead of your scheduled appointment time, you will incur a “no-show” fee.  This fee is the responsibility of the patient, not the insurance carrier. Cancellation penalties are expected to be paid in full before or at the time of your next appointment.

  • The penalty fee for a no-show to a follow-up appointment is $25.
  • The penalty fee for a no-show to a procedure appointment is $40.

Appointment Reminders: To help you avoid cancellation penalties, we will call you to confirm your appointment 24-hours in advance. We must have a valid daytime phone number for you to confirm your appointment. Please advise the office staff of a preferred phone number for appointment confirmations.

Return Phone Calls: If you call our office with a question for the medical staff, it is likely you will not hear back until after 3 pm.  Our office staff are not permitted to interrupt medical staff while they are seeing patients. Please be patient. We will return your call as soon as possible.

Scheduling an Appointment

Thank you for choosing Pain Care Center.

Patients must have an appointment. We do not accept walk-ins.

We cannot schedule an appointment without a physician referral.  Please have your physician contact our office to make the referral.  Once we receive the referral, we will contact you by phone to schedule an appointment.

Please help us gather the necessary information to begin your treatment plan as soon as possible.  Our office will need the following information from your referring physician:

  • A list of all medications you are currently taking or have taken during the past year
  • Radiology reports: X-ray, MRI, CT
  • EMG reports
  • Lab results
  • Previous pain clinic records
What to Expect

Please be sure to bring a photo ID, insurance card, new patient packet, and medications in original bottles to your appointment.

Arrival Time: Please print and complete the New Patient Packet and bring it to your first appointment.  If you have completed the forms, please plan to arrive 30 minutes before your scheduled initial consultation. If you have not filled out the forms or didn’t bring them with you then please arrive one hour before your appointment time.

Check-In: When you arrive to our office, please check in with the receptionist. A member of our team will ask you to complete:

  • Additional registration information
  • Medication agreement form
  • Medical records release

Initial Consultation: During the initial consultation, we will do a thorough investigation of your medical concerns.  We will review your medical history and prior treatments for your pain, perform a physical exam, discuss treatment recommendations, and address your questions and concerns.

Beginning Treatment: Your treatment plan will guide the next steps.  It is possible that formal treatment will not be started during your first visit.  There are several reasons why there could be a delay:

  • Your treatment includes a specific injection.  This is considered a procedure, which must be scheduled for another day.
  • Past medical records are not adequate and further collaboration with your primary care physician is necessary prior to moving forward.
  • Concern of the possibility of illicit drug use.
  • Additional testing, such as urine, blood or radiologic tests, required prior to starting therapy.
Insurance

Unity Healthcare and its affiliated facilities participate in many insurance plans. Participation varies and is subject to change. Please verify with your Unity provider that your health plan is accepted.  Also, access to a Unity Healthcare provider may require authorization by your primary care physician or your managed care plan.

Follow My Health is an electronic medical record that provides you with a secure and convenient way to manage your personal health care online. This free self-service health management tool can improve interactions with your doctor’s office, document important health care information, and allow you to make more informed decisions about your own health.

The patient portal is the fastest way to request and process medication refills.

Prescription Refills

How to Request: Prescription refill requests take time to process. There are two ways to request a refill:

  1. The most efficient way is a request through the patient portal.  It is much faster for our medical providers to complete tasks within the on-line medical records than through a phone call requests.
  2. Call our office at 765.807.7988 to request a refill. Please note that you must call to make the request.  Another person cannot call for you. If you call after hours, please leave the following information on the machine:
  • First and last name (spoken clearly and spelled out)
  • Phone number that we can use to call you back if there is a problem
  • Prescription requested with strength and date your medication will be gone.

Timeframe: It can take up to two business days to process a refill request so do not wait until you are out of medication. Please plan accordingly so you are not in pain due to a lapse in medication.

Picking up Script:  You may call our office 3 business days after your request was made to make sure the written prescription is ready for pick up.  Depending on the script, some patients may be allowed to appoint another person to pick up the script by calling ahead to the front office with the name and relationship of the pick-up person. The appointed person must show picture identification at the time of pick-up.

Weekend Refills: We do not process prescription refills during the weekends.
