Innovations Pain Management Group Relocates Office

Unity Healthcare is pleased to announce that Tonia Kusumi, MD has moved to her permanent location. Innovations Pain Management Group's new address is 3738 Landmark Dr, Suite A on the corner of South Creasy and McCarty Lane, across from Bennett's Greenhouse.

Dr. Kusumi is an interventional pain management physician who has been practicing in both Lafayette and Logansport since 2007. Innovations Pain Management Group will continue to participate in the many health plans which Unity Healthcare offers.
